About this starter form
Teachers and organizers can use this designer to prepare blank registration forms for a school event, workshop or community program. The starting fields cover the participant name, contact details, event session and participation type. Participants complete the printed form by hand.
Adapt the form to your event
Rename or remove fields, add writing space and create checkbox choices for sessions or attendance types. Use section headings to keep groups of questions clear. A longer form automatically continues onto another page.
Select your layout
Add your organization's name and a local logo. Choose one of 25 Classic, Smart, Digital or Islamic designs and view changes immediately. You can include a text or image watermark, with 10% opacity by default and an adjustable range from 5% to 25%.
Download and distribute
Select A4 or Legal paper, download the blank PDF and print copies at Actual size / 100%. Give the hardcopies to participants. This is not a ticketing or booking service, and the designer does not collect completed registrations.
