About this starter form

This admission form designer is for teachers and school staff who prepare printed forms for students. Start with ready-made field labels for the student name, date of birth, class, guardian details and address. The output is a blank form: students fill the printed hardcopy by hand.

Customize the questions

Open Fields to rename, reorder or remove the starting labels. Add writing lines, answer boxes, date boxes, checkbox choices or section headings. Give longer answers more writing lines. Layouts continue onto additional pages when needed.

Add your school identity

Enter your school name, address and instructions. Choose a local logo image and optionally add a text or image watermark. Watermark opacity starts at 10% and can be adjusted from 5% to 25%. Your images and edits stay in the current browser tab.

Choose a design and paper size

Select one of 25 Classic, Smart, Digital or Islamic designs. The live preview updates without losing your fields. Choose A4 or Legal paper, review each page and download the PDF. Print at Actual size / 100% on matching paper and distribute the copies to students.
